Aproximadamente 80% das TVP são clinicamente silenciosas2 > 70% das EP fatais só são detectadas post mortem1 O TEV é responsável por 10% da mortalidade hospitalar3 A embolia pulmonar (EP) é responsável por duas vezes mais mortes do que o conjunto4: SIDA Cancro da mama Cancro da próstata Acidentes rodoviários There is a very strong association between pulmonary embolism (PE) and deep vein thrombosis (DVT): - 90% of pulmonary emboli are the results of DVT - A recent study showed that 82% of patients with acute PE had detectable DVT at the time PE was diagnosed. - Silent PE occurs in more than 50% of patients with symptomatic proximal DVT. As the venous clot in the leg grows it extends along the vein. DVT can develop into a chronic condition in which damage to the valves in the veins results in swelling of the leg, pain and venous hypertension (known as post-thrombotic syndrome - PTS) Parts of the clot can also detach, travelling in the blood to block vessels in the lungs, subsequently causing pulmonary embolism (PE). If less than 50% of the pulmonary circulation is blocked, symptoms are generally limited to shortness of breath or are absent altogether. Em Moçambique com o inicio das artroplastias como rotina e outras cirurgias “major” principalmente nos membros inferiores, esta ameaça é de prever. Desde 2005 ja tivemos casos de mortes devidos a tromboembolia e a partir de certa altura desde 2009 a profilaxia com heparina é mandatória no nosso departamento. Therapeutic window Trombose Hemorragia An anticoagulant should ideally have a wide therapeutic window. Vitamin K antagonists are unpredictable because they have a narrow separation between the dose that significantly reduces thromboembolic events and the dose at which there is an excessively increased risk of bleeding (upper panel). Ideally an anticoagulant should have a wide therapeutic window i.e. a wide separation between the dose that significantly reduces thromboembolic events and the dose at which there is an excessively increased risk of bleeding (lower panel). New compounds are being developed with the aim of producing an anticoagulant with a broad therapeutic window. The therapeutic window for rivaroxaban was investigated by studying safety and efficacy of a wide range of doses in the phase II studies. With rivaroxaban, there appears to be a wide separation between the dose that significantly reduces thromboembolic events and the dose that leads to an excessive risk of bleeding. This suggests a wide therapeutic window for rivaroxaban, facilitating dosing and removing the need for monitoring. Dose
