5 O QUE É EXACERBAÇÃO? This document is widely accepted as the most up-to-date guidance on COPD management.1 The major symptoms of exacerbations are increased breathlessness, increased cough and/or sputum, and changes in colour and/or tenacity of sputum.2 Exacerbations may be classified based on symptoms or on the type of medication used to treat the exacerbation.3 Patients may take several weeks to recover from severe exacerbations.4 Exacerbations are commonly associated with bacterial and/or viral infections of the airways and worsening air pollution.1 Exacerbations are associated with increased levels of inflammatory markers, such as IL-6, in the airways and systemic circulation.5-8 References Global Initiative for Chronic Obstructive Lung Disease. Bronchial biopsies and sputum samples were obtained from 11 subjects with chronic bronchitis examined during an exacerbation, and from 12 subjects with chronic bronchitis examined under baseline conditions. Lobar bronchial biopsies were assessed using histochemical and immunohistochemical techniques, and sputum was examined for differential cell counts of leukocytes. The number of neutrophils was significantly increased during exacerbations compared with during the stable state (p<0.01). The number of other inflammatory cells also increased during exacerbations, providing clear evidence that exacerbations are associated with an increased inflammatory response. In this study, sputum samples from 57 patients with moderate to severe COPD were taken during a stable period. Patients who had experienced ≥3 exacerbations during the previous year had higher levels of inflammatory cytokines (IL-8 and IL-6) in sputum samples, compared with patients who had experienced ≤2 exacerbations. Many patients with COPD suffer from symptoms associated with chronic bronchitis, including chronic cough and/or sputum production. Recent studies have shown that patients with chronic cough and sputum have more than twice the number of exacerbations compared with patients without these symptoms.1,2 Among 2,500 patients in the COPDGene Study, those with symptoms of chronic bronchitis had significantly more frequent and more severe exacerbations than patients without these symptoms, even though both groups had similar FEV1 levels.2 Symptoms of chronic cough and sputum are also associated with steep lung function decline3 and increased mortality.4,5 Smokers with symptoms of chronic bronchitis have increased inflammation compared with asymptomatic smokers.6 References Burgel PR, Nesme-Meyer P, Chanez P, et al. Data from the ECLIPSE cohort illustrated that exacerbations become more frequent and more severe with increasing severity of COPD. Patients with two or more exacerbations during the year were considered to have frequent exacerbations. Exacerbation rates in the first year of follow-up were 0.85 per person for patients with GOLD stage II COPD, 1.34 for patients with GOLD stage III, and 2.00 for patients with GOLD stage IV. Overall, 22% of patients with stage II disease, 33% with stage III, and 47% with stage IV had frequent exacerbations in the first year of follow-up. The single best predictor of exacerbations, across all GOLD stages, was a history of exacerbations in previous year. The frequent-exacerbation phenotype appeared to be relatively stable over a period of 3 years and could be predicted on the basis of patients’ recall of previous treated events. The ECLIPSE data suggests that the ‘frequent exacerbator’ is a distinct sub-group of patients who may be easily identified (based on patient recall) and targeted with specific exacerbation prevention strategies. A cohort of 304 men with stable COPD were recruited and followed for 5 years. Severe acute exacerbations were defined as symptomatic changes requiring modification of regular medication and hospital treatment (either a visit to the emergency room or admission). Patients were divided into 3 groups based on the number of severe acute exacerbations experienced during the 5-year observation period. During the study, 53.6% of patients experienced no exacerbations, while 34.5% experienced 1-2 exacerbations and 11.8% experienced ≥3 exacerbations. Generally, patients with frequent exacerbations had later stages of COPD (GOLD Stages III to IV), although around 14% of patients who experienced ≥3 exacerbations had moderate COPD (GOLD Stage II). The rate of mortality increased significantly with the frequency of exacerbations, with patients who experienced ≥3 exacerbations having a risk of death 4.3 times greater than patients who had no exacerbations. Paciente com exacerbações frequentes Aumento das exacerbações Aumento Da Inflamação Queda na qualidade de vida Aumento da mortalidade Aumento das hospitalizações Progressão mais rápida da doença Speaker notes Some patients with COPD are particularly susceptible to frequent exacerbations.1,2,3 Cohort studies have reported evidence of a phenotypic frequent exacerbator, group3,4 While exacerbation frequency is linked to disease severity, it is important to consider that not only patients with severe COPD are frequent exacerbators.2 Exacerbations are associated with increased inflammation, which persists after the attack and affects the length of recovery period.5 In addition, patients who suffer from frequent exacerbations have increased airway inflammation in the stable state.6 Frequent exacerbators display worse quality of life,1,7 faster decline in lung function,1,2,7 and higher mortality rates than patients with less frequent exacerbations.1,2,8,9 Managing exacerbations, including reducing their frequency and severity, is a major goal of COPD disease management in the GOLD Global strategy for the diagnosis, management, and prevention of COPD.10 References Wedzicha JA & Seemungal TA. Níveis elevados de AMPc intracelular reduzem a inflamação 3´-5´ AMPc Speaker notes Roflumilast is a highly selective inhibitor of phosphodiesterase 4 (PDE4), an enzyme which regulates the infiltration and /or the function of inflammatory cells via the second messenger cyclic AMP (cAMP). Low levels of intracellular cAMP lead to the activation of inflammatory cells. PDE4 catalyses the breakdown of cAMP to its inactive form, leading to increased inflammation. By inhibiting the PDE4 enzyme, the degradation of cAMP is prevented and its elevated levels are maintained, reducing the pro-inflammatory functions of the cells. 1,2 Daxas® is the first PDE4 inhibitor approved for treatment of COPD. 35 Fosfodiesterase 4 (PDE4)
A enzima Fosfodiesterase 4 (PDE4) está presente em diversas células inflamatórias e estruturais. A PDE4 desempenha papel importante nas células inflamatórias envolvidas na DPOC Inibires da PDE4 ( ex Roflumilast) eleva os níveis intracelulares de AMPc e reduz a atividade inflamatória de neutrófilos, linfócitos CD8+, e macrófagos. Os inibidores da PDE4 são uma nova alternativa terapêutica para o tratamento da DPOC Slide 35
