ANAMNESE Idade: 03 anos. Sexo: Masculino. Queixa: “Caroço” no joelho que aumenta de tamanho há dois anos. Nega dor e limitação funcional. Realizada ressonância nuclear magnética (RNM) do joelho esquerdo.

8 HIPÓTESES DIAGNÓSTICAS
Osteocondrose sinovial solitária. Hemangioma. Sinovite vilonodular focal. Hemangiopericitoma.

9 BIÓPSIA Tecido conjuntivo denso e hialino com extensas áreas de degeneração mixóide, sem atipias. Focos de inflamação.

11 FASCEÍTE NODULAR INTRA-ARTICULAR
Até Março de 2014, 18 casos de fasceíte nodular intra-articular foram relatados e, em nenhum dos casos, esta patologia estava nos diagnósticos diferenciais após a avaliação clínica e radiológica, sendo necessária a biópsia, assim como nosso caso. Mais comum são os membros superiores – principalmente antebraço (36%). 20% envolve a região da cabeça e pescoço. 20% o tronco. 16% os membros inferiores. Ocorre mais em adultos jovens, entre 20 e 40 anos, sem predileção por sexo. Causa desconhecida, mas pode ter história de trauma.

12 Para seu diagnóstico o estudo anátomo-patológico é necessário.
EXAMES DE IMAGEM RESSONÂNCIA NUCLEAR MAGNÉTICA: Intensidade de sinal inespecífica. T1: imagens com iso ou hipersinal em relação ao músculo. T2: imagens com hipersinal em relação ao músculo e à gordura. Lesões redondas ou ovais e, geralmente, bem circunscritas. Após o contraste Realce difuso. Para seu diagnóstico o estudo anátomo-patológico é necessário.

O diagnóstico diferencial de uma lesão intra-articular no joelho de um paciente pediátrico é ampla Sinovite vilonodular pigmentada - tumor intra-articular mais comum . Lipomas. Hemangiopericitomas. Fasceíte nodular. Osteossarcoma parosteal. Tumor desmóide Neurofibromas. Sarcomas fibromixóides. O sucesso do tratamento tipicamente consiste na excisão marginal da lesão, podendo, em 1% dos casos, haver recorrência.
